Job Description

Job Summary: We are looking for a weekend option skilled and experienced Licensed Vocational Nurse (LVN) Care Navigator to join our team at Bay 9 Holdings LLC dba Edgemere located in Dallas, Texas. The Licensed Vocational Nurse (LVN) Care Navigator is responsible for collaborating with physicians and an interdisciplinary team to provide safe and effective transitions of care to residents admitted to our health centers for Medicare Part A short-term stays. This position also provides support to family members as well as education and support to our interdisciplinary team members. This role is responsible for demonstrating a commitment to customer service and care management through resident focused initiatives that support the mission of the community. Compensation & Benefits: This is a Part-time/PRN, hourly position with a starting salary of $25.00+ per hour.   • Direct nursing services to all residents on assigned residence under the supervision of a registered nurse.
  • Serve as a resident/family advocate to facilitate positive outcomes for the resident, which are achieved through collaboration with the resident/family as well as healthcare providers participating in the resident’s plan of care.
  • Empower the resident to problem solve to achieve desired goals, to be their own advocate, and to be an active participant in the determination of their own plan of care.
  • Attend Interdisciplinary Team Meetings, review the resident's medical record, and gather clinical information from the current care delivery team, and the resident/family, to assess progress toward goals, discharge, and anticipated length of stay.
  • Work with interdisciplinary team and primary care providers to facilitate and streamline the resident move-in and move-out process.
  • Introduces an accurate and thorough report to team members prior to move-in of the resident to ensure the continuity of maximum resident care.
  • Verify all physician orders for medications, care and treatment are received timely and are transcribed accurately in electronic records and treatment plan.
  • Encourage resident/family engagement and involvement in care by providing education regarding the resident's medical condition, disease and symptom management, and referral sources and community resource options.
  • Follows established protocols to document the resident’s transitions of care, including emergency room visits and hospital readmissions. Assures care plans and move-out plans are continually reviewed for updated and accurate data that represents the individuality of the resident.
  • Communicate clinical and personal transitional goals throughout the post-acute care continuum to promote optimal recovery.
  • Communicate to the receiving provider as the resident transitions from one level of care to the next (includes bidirectional admissions or discharges).
  • Enhance the transition of care process through clear and timely communication of the resident’s plan of care to both the sending and receiving healthcare providers until the transition is complete.
  • Provide telephonic and/or onsite support to residents who are transitioned to the next level of care and conduct resident survey within 72 hours post transition. All pertinent information collected will be reviewed and assessed with appropriate healthcare team providers.
  • Follows up on resident and family concerns as needed in a timely manner.
  • Comply with applicable federal, state, and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures.
  • Maintain regular and punctual attendance at work and meetings.
Requirements:
  • Licensed Vocational Nurse graduate from an accredited nursing program and other licensure required by state regulations.
  • Nursing diploma or associate degree in nursing from an accredited nursing program. Bachelor's degree is preferred.
  • Previous care management experience is preferred
  • One year of nursing experience in a long-term facility is preferred. Supervisory/leadership in a long-term care facility preferred.
