The Opportunity:

As an Advanced Practitioner, you'll be a vital part of an integrated care team, working alongside a Registered Nurse and a Social Worker to improve patients' quality of life and slow the progression of kidney disease.
You'll have the opportunity to build meaningful relationships with patients, providing them with the support and guidance they need to live healthier lives.
What You'll Be Doing:
Conduct comprehensive assessments, including annual wellness exams, in patients' homes and virtually.
Counsel and educate patients and families about available benefits and programs.
Maintain accurate and current patient records.
Coordinate care with primary care providers, specialists, and ancillary services.
Deliver evidence-based, timely care to reduce hospitalizations and improve quality of life.
Prescribe medications, order tests, and collaborate with the patient's physician.
Educate patients about preventive monitoring and schedule diagnostic testing.
Assist patients with accessing educational resources.
Participate in integrated care team meetings.
Develop and implement personalized treatment plans.
Follow up and evaluate patient status.
Perform other duties as assigned.
What You Bring to the Table:
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work autonomously and without direct supervision.
Access to transportation, a valid driver's license, and car insurance.
Proficiency with medical instruments and equipment.
Knowledge of computer-based data management programs and medical records systems.
Excellent communication skills.
Understanding of federal and state regulations, including HIPAA and OSHA.
2+ years of direct patient care experience.
Managed Care/IPA/Health Plan experience.
Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home.
Bilingual skills (English/Spanish) highly preferred.

This position is based in Harrisburg/Hershey, PA.
This is a remote role within the state, but requires attending periodic training/meetings outside the state.
